1. ASSUMPTION OF RISK

I, the undersigned parent or legal guardian, understand that participation in Pamper Pouch mobile spa services—including but not limited to foot soaks, nail painting, mud masks, makeup application, and other spa-related activities—may involve the use of cosmetic and skincare products.

I acknowledge that I am voluntarily allowing my child(ren) and their guests to participate and assume all risks associated with these services.

2. ALLERGIC REACTIONS & SKIN SENSITIVITY

I understand that Pamper Pouch uses commercially available cosmetic and skincare products. I acknowledge that:

• My child(ren) and guests participating may have unknown sensitivities or allergies.

• Pamper Pouch is not responsible for any skin irritation, allergic reactions, or

  adverse effects resulting from the use of products.

• It is my responsibility to inform Pamper Pouch in advance of any known allergies,

  sensitivities, or medical conditions.
  I accept full responsibility for any reactions that may occur.

  3. MEDICAL DISCLAIMER

  Pamper Pouch does not provide medical advice or treatment. Services are for entertainment and cosmetic purposes only. I confirm that my child(ren) and participating guests are in suitable health to participate.

  4. SUPERVISION REQUIREMENT

  A responsible adult (age 18 or older) must remain present at all times during the event. Pamper Pouch staff are not responsible for supervising children beyond the scope of spa services.

  5. LIMITATION OF LIABILITY

  To the fullest extent permitted by law, I agree to release, waive, and hold harmless Pamper Pouch, its owner(s), employees, and contractors from any and all liability, claims, damages, or expenses arising from:

• Participation in services

• Use of products

• Allergic reactions or injuries

• Accidents or incidents occurring during the event

6. PROPERTY & DAMAGES

The client agrees to provide a safe and appropriate environment for services. Pamper Pouch is not responsible for:

• Damage to clothing or personal items during services

• Pre-existing conditions of skin, nails, or hair
  The client may be held responsible for any damage caused to Pamper Pouch equipment due to negligence or misconduct.

  7. PAYMENT TERMS

• A deposit is required to secure the event date.

• The remaining balance must be paid in full on or before the date of the event.

• Failure to complete payment may result in cancellation of services.

  8. CANCELLATION POLICY

• Cancellations made less than 48 hours prior to the scheduled event will result in

  forfeiture of the deposit.

• Rescheduling requests are subject to availability and are not guaranteed.

  9. PHOTO & MEDIA RELEASE (Optional)

  ☐ I give permission for Pamper Pouch to take photos/videos during the event and use them for marketing purposes.
  ☐ I do NOT give permission.
